The sample pages need to be representative of the quality of the whole manuscript, rather than an example of the best work, and context is not important. Any format is fine: .doc, .docx, .rtf, PDF etc.
If you are just completing your first draft and have not yet had anyone else read over your work, you may not yet be ready for a professional editor. I recommend that you do as much work on your manuscript as you can yourself before you consider hiring an editor.
